Whether to remove redundant graphics state save/restore pairs

A pair of graphics state save and restore operators (q/Q) whose restore is immediately followed by another restore saves a state that no drawing operator observes. Removing such pairs does not change the page's appearance. Deeply nested content produced by some applications is reduced to the nesting depth it actually needs, which keeps documents within the limit of 28 nested graphics states required by PDF/A-1.
